  1. Start with the document type. Decide whether you need a prescription pad, letterhead, admission form, handout, label or folder.

How to Customize and Print Your Template

  1. Open the template page and download the file. Templates are made for Microsoft Word, and most can be opened in other word processors too.
  2. Replace the placeholder text with your name, qualifications, registration number, clinic name, address and contact details.
  3. Add your logo if you have one, and adjust fonts, colors and spacing to suit your practice.

What a Good Prescription Template Includes

Requirements differ by country, state and professional body, so always confirm your local rules. In general, a clear prescription layout has space for:

  • The prescriber’s name, qualifications, registration or license number and contact details
  • The clinic or practice name and address
  • The date and the patient’s name, age or date of birth
  • The Rx symbol with room for each medicine, strength, dose, frequency and duration
  • Instructions, warnings and follow-up notes
  • The prescriber’s signature
